7. Purchase Date (must be within the promo period)
8. Model Name
9. Qty. purchased (if the customer purchased more than 2 units)
10. Product Serial Number
11. Upload all the required attachments:
    1. Proof of purchase (must be clear and readable)
    2. Valid government-issued ID
    3. Photo of the product serial number (can be found at the back or the bottom of the actual monitor)
    4. Photo of Company ID and authorization letter*

          *If the purchase was made under a Company or Organization, the authorized representative must provide his/her company           ID together with a company authorization letter signed and certified by the company’s signatory. This letter should state           and assign him/her to join the raffle and claim the prize on behalf of the company or organization.

8. The customer will receive a confirmation email for the successful redemption.

9. AGON by AOC Philippines reserves the right to disapprove submitted redemption if the customer is unable to meet the conditions, or has incomplete requirements, outlined in this promo’s mechanics.

10. AGON by AOC Philippines reserves the right to suspend the release of the giveaway Anniversary Bundle should misrepresentation or fraud be proven.

11. After verification, customers will be notified via phone call, registered mail, and/or email, in no particular order, for final verification of the delivery details.

12.The giveaway Anniversary Bundle will be dispatched within 10-15 working days, free of charge, within 60 days upon receipt of official notice via registered mail and/or email to the customer’s registered address.

          12.1 Customers will be contacted by one of the staff under AGON by AOC Philippines to confirm the delivery details.

         12.2 Upon receipt of the item, the customer shall provide a clear photo together with the giveaway Anniversary Bundle and              shall send it to the AGON by AOC Philippines email address marketing@mmdsg.com.ph and/or official Facebook page

          www.facebook.com/agonbyaocph to confirm the successful arrival and receipt of the item.

13. The customer has sixty (60) days from receipt of notification to claim their giveaway Anniversary Bundle. After the 60-day claiming period, the unclaimed giveaway Anniversary Bundle will be forfeited in favor of AGON by AOC Philippines with the prior approval of DTI.

14. Employees ​​of the brand, distributor, reseller, sponsor, advertiser, or employees under a contract of service and who, by the nature of the service they render may have direct and personal knowledge of this promotion including their relatives up to 2nd degree of consanguinity or affinity are disqualified to join the promo.
